This 4-day-old neonate born at 25 weeks of gestation suddenly deteriorated, becoming hypoxic and bradycardic while ventilated. Breath sounds were reduced bilaterally. Pneumothorax was suspected but cold light transillumination was inconclusive and an urgent chest radiograph was obtained (figure 1). Answer the following questions based on the chest radiograph appearances.
